1. There are no omissions of any kind, and no misrepresentations or falsifications, and the above answers are true and accurate and are made in good faith. I understand and acknowledge that any omission I have made or misrepresentation or falsification may be grounds to discontinue further consideration of my application, or call for my immediate dismissal from the Legislative Internship Program;

2. I understand that I am responsible for arranging reliable transportation, to and from Hartford, on the scheduled days I selected above;

3. I understand that the scheduled days I selected above are a factor in the selection process and that any changes sought in this regard may jeopardize my ability to participate in the program;

4. I understand that it is my responsibility to provide the on-campus advisor to the program the materials outlined above (step three of the application process), prior to the November 1st deadline, in order to complete my application.
