Welcome to the Connecticut General Assembly 360° assessment process. You have been chosen to provide feedback to someone participating in our leadership development and succession management program. The assessment is designed to help the person you are rating to improve his/her overall effectiveness and leadership skills.
Your responses are ANONYMOUS and will be grouped with the responses from others to provide feedback to the participant. The participant will use the feedback received to prepare an action plan for his/her personal development.
INSTRUCTIONS:
This survey consists of 50 statements relating to the leadership qualities of the person you are rating. You are asked to rate this person on a scale ranging from 1 – 5. Please indicate which number best describes the person you are rating. See the guide below for a description of the rating scale.

Be candid. Try to avoid making all your ratings in the middle or “average” part of the scale. In order for the feedback to be useful to the participant, it must suggest clear-cut themes, as well as development areas.
Examine each item individually. Rarely will an individual be strong or weak in all areas. Most of us have both strengths and weaknesses; try to allow these to come through in your ratings.
Help the participant to better understand his/her professional style by rating items openly and honestly.
Be honest. Remember that in order for the results to be useful to the participant, you must be particularly straight forward in your ratings. If you have not had an opportunity to observe the person demonstrating the indicated behavior, use the “Don’t Know” category by selecting the “x”.
Please remember this feedback is used exclusively as a development tool. This participant is asking for your honest feedback so he/she can develop the skills necessary to become a successful leader.
